Facilities Use

Lafayette School District facilities and fields are available for community use throughout the year. Facilities Use Applications are accepted beginning on May 15th every year for the subsequent school year.

Please submit the following to [email protected] to apply:

1. Facilities Application and Permit Form (2 Pages)
2. Signed Facilities Agreement Compliance Form
3. A current Certificate of Liability Insurance naming the Lafayette School District as the holder in the amount of $5,000,00
