Facilities Use

Facilities Use Applications are accepted beginning on May 15th every year for the subsequent school year.


*All facilities use will be subject to the public health guidance related to the

COVID-19 pandemic that is applicable at the time of the inteneded use, and

a facilities permit agreement may be modified or canceled in order to comply with such

guidance.


Please submit the following to apply:

 

(1) Facilities Application and Permit Form 2021-2022

(2) Signed Facilities Agreement Compliance Form

(3) A current Certificate of Liability Insurance naming the Lafayette School District as the holder in the amount of $5,000,000

 

Note: A key deposit of $2,500 per key will be required for key distribution.
